EstherGM. Love your podcast. Especially Dr. Topf. I am a female cardiology NP. My 96 YO pt is active with daily socializations. He has significant diastolic heart failure with pitting and weeping edema. His Cr. 1.7. Brought him down to 1.4 resolved his edema with the help of Jardiance. He is living his best life until his nephrologist called me ridiculous and taken him off SGLT2. My question although SGLT2 (class A1) is effective and not studied in a 96 YO- am I ridiculous to start the med in a 96 YO.

Nawar Al janabiFor patient with HFrEF and started on GDMT, do we need to keep them on full GDMT treatment after EF improvement? Or we can start stopping some of them gradually to decrease side effects. Sorry if the question was addressed before.
