The vratham is austerity for 1 mandaalam. Climbing the 18 steps is only those who wears mala, and reaches with irrumudi, and not just vratham, as pilgrims can perform austerity and  still not wear the mala and the irrumudi.. Also one can reach the sanctum sanctorum through the west side for people without irrumudi. Also per mythology Ayappa promised to marry Maligathamma the day there is no sharam (arrow) at Sharamkuthi meaning no Kanni swami has reached that year. So there is a ritual where the goddess goes on top of an elephant upto sharamkuthi and returns to wait for the next year.
